Our aim is to make the referral process as smooth as possible, and we will work closely with you to ensure a seamless transfer of all necessary records.

***If transferring a pet for hospitalization, a full work-up MUST already be complete by the referring veterinarian. Prior to their arrival, you must inform your client that payment is due at the time of service, and that we do not accept Care Credit.

If a work-up has not yet been done, DO NOT COMPLETE THIS REFERRAL FORM. Contact our hospital instead to ask if we are currently able to see this as a new case. Emergency/Work-in fees will apply.
